Diabetes Positive Approach Newsletter #1402 on Exercise

Hey you guys, how's our self-image coming along? Great, I suppose because we are all here for each other. How can it be otherwise? But you know what? The best and most effective way to improve our self-image is to join a club or an organization that has worthwhile purposes. And we have that in our own little club, don't we, The Diabetes Positive Group. There's a ring to it, isn't there?

This is great for our next topic which is Exercise. Some of us will not want to do that but we will push each other. We might experience some difficulty getting people up but know that it is not necessary to become an exercise pro. We can just start somewhere and pretty soon as our confidence develops, I bet we will have a hard time getting you to stop. Exercise and Diabetes

Exercise is one of the three fundamentals in treating diabetes. The other two are food and medications but among these, exercise is cheap, easy, convenient and results in better blood sugar control. On top of this one cannot deny the fact that after the exercise, one usually feels well and healthy. How does this come about?

You see, whenever we use our muscles and that’s a given when we exercise, we burn both the glucose and the fatty acids. The beta cells in our pancreas can feel this so they relax the production of insulin giving them a much needed break. The lower insulin level alerts the liver to release its glucose reserves into the blood because the muscles need the energy for the exercise.

As the exercise continues, the liver turns the fats, lactic acid and amino acids into glucose to give the muscles what they need. This tells us that exercise is the best diabetes treatment because not only does it lower the blood sugar and fatty acid levels but also it gives the pancreas some time off from its work. You won’t suffer from hypoglycemia because exercise won’t lower the blood glucose to way below, the way some treatments may. But there are some risks, so listen up!

Heart Disease Risk

Before going into an exercise plan, consult with your doctor as it is imperative to know if heart disease is present as this is common, as much as 50%, among the PWD (people with diabetes). The doctor will consider your age, how long you have had diabetes, the family history, the blood fats and blood pressure and the presence of protein in the urine.

Undergo an exercise tolerance test if needed where they will make you walk on a treadmill and measure your heart’s response to stress. Don’t lose hope though if you find yourself in the positive territory because you can still exercise following some guidelines your diabetes care team will provide you. Besides, there may be medications that can lower this risk.

Hypoglycemia

This is a concern for those who take insulin or some form of medication. If you’re in this category, you will have to do glucose testing to monitor how your body responds to exercise or any type of activity so you will be prepared for the low blood sugar that might come for a visit.

What some people do in such a case is drink orange juice or some other thing to treat the low making their blood sugar go high. This leads them to take extra medication or insulin only to continue the roller coaster ride with another low. It is very frustrating, isn’t it? What is worse is that they may stay away from exercise thinking it is not worth it when it leads to such confusion and scary scenario.

They are missing the point that testing the blood sugar more often will help them understand better the body’s reaction to exercise. With this more frequent blood glucose testing, they will be able to make better adjustment to the other two cornerstones of diabetes treatment: medication and food.

Diabetes Complications

Believe it or not, there are complications that may make it unwise to increase the physical activity. For example, any activity that requires one to strain and lift weights may just do more harm than good for those who have high blood pressure. In fact the aforementioned activities will just exacerbate hypertension. So make sure the blood pressure is under control before increasing the activity level.

Another medical condition that can be harmed by increased exercise is retinopathy. Exercise that includes straining will increase the pressure on the eyes’ blood vessels which may already be weak to begin with. So before starting an exercise program, get your eyes checked for the presence of retinopathy and do this annually.

With nerve problems in your feet, you may not be able to feel injuries in the form of blisters and sores. Get to the doctor to have this checked before embarking on a rigorous exercise program. You should also examine your feet every day and wear appropriate shoes for exercise. It does not mean you cannot exercise. Just build your own program to suit your needs and follow the guidelines for your specific situation.

Guidelines for Exercise

Guidelines can help you ease into increased level more safely. Monitor your body’s response and request your health care to help you with this. After you have been screened through treadmill stress test, eyes and foot examination and protein in the urine, you are ready to choose appropriate activities or exercise.

Type of Activities

Choose activities that you enjoy so you don’t get bored to the point you will make excuses to stop. Indulge in a variety of exercises, some by yourself, others with a group. Some can be outside in the open air like tai chi and others inside the house. You could have more moderate ones when you have time and lighter type of exercise when you are pressed for time.

Best Time to Exercise

The best time to exercise? Anytime at all. It is hard enough for some people to find time to exercise. It will even make it harder if we dictate the best time to do it. For one thing it is an individual thing. We have different schedules for work and play so only each person can work out a plan to suit his own time frame.

The more important question is on what to remember when you start an exercise plan. Test the blood sugar before and after the exercise. It is because when you do this, you will know how exercise affects your blood sugar level. It is a balancing act but we have to tract how the body responds to exercise, food, medication and even stress. This is the way we can make the necessary adjustment for optimal health.

Intensity of Exercise

Do warm up first by mild stretching and marching around the room. This will help the heart get ready for the exercise. Since most PWD have brittle tendons due to years of high blood sugar level, it is good to do some gentle stretching to warm up in order to avoid trouble with muscle and tendons.

Glucose and Exercise

The next question is how much to exercise. The recommendation from the American Diabetes Association is not to exercise when the fasting blood glucose level is higher than 13.89 mmol/l (250 mg/dl) and ketone is present. We also have to be careful when the ketone is not present when the reading is16.67 mmol/l (300 mg/dl). ADA also recommends for us to eat a snack before exercise when the level is below 5.56 mmol/l (100 mg/dl ). Then test again after 15 minutes.

Do the activity or exercise after eating as this will help bring down the increased blood sugar after a meal. If you take insulin, know the peak times so you can plan your activities appropriately. Avoid the activities when the insulin is peaking to prevent hypoglycemia. Bring glucose tablets if you are at risk for hypoglycemia which if this occurs, stop the exercise right away and take the glucose tablet.

If you have repeated hypoglycemic episodes when you increase your activity, you should tell the doctor as he may have to change your medication. Monitor your blood glucose while exercising to see how the activity affects you. Check it before you start and every half hour during exercise.

7. Q&A: I get too busy all day long. How can I fit in an exercise plan?“

Answer: Being busy all day long is good. You are probably burning all kinds of calories every day. In fact, the surgeon general said some activities will burn 150 calories a day or around 1000 calories a week. How do you like that? You may be wondering what those activities are. Here are some examples:

Shoveling snow for 15 minutes, climbing steps for 15 minutes, walking 2 miles in 30 minutes, gardening for half an hour to 45 minutes, bicycling 4 miles in 15 minutes, pushing a stroller for one a half miles for half an hour are some of the activities. They are heart-pumping ways and burn calories to help you get fit.

All that exercise tired me out. Truth be told though, I feel great after the end of an exercise session. It is only at the start that I feel, “Yikes, not again! Why do I have to do this? But then comes the end of the session and I feel like I can do anything. I can even look at Prime Minister Harper in the eye.

Don't you find sometimes it is hard to look at other people in the eye? There are some people who cannot look directly at someone they are having conversation with. Some have such self-image they feel they are not worthy to look at some people directly. Well, we are not like these people because we have been developing our self-image. Yay!

Diabetes Positive Approach Newsletter 1401 - Eating Out

First, I must say a one-sentence explanation for this post. This is really a newsletter for my subscribers as I have migrated from SBI, my previous host to Blogger, but new readers are more than welcome to join us. I am so excited about this topic for this newsletter. It is eating out. Yay, that's right down my alley. But in my excitement we should not forget that we are building our self-image. Do you know how else we can improve our self-image? It's by studying how others failed and survived!

I can give you a lot of examples. There's Michael Jordan who said he failed so many times and yet he's known as Mr. Basketball. There's Henry Ford, a wealthy man, but when he was 40 years old, he was broke. There's Albert Einstein who failed his math courses. Can you believe it? That genius failed at Math? Oh, there's still hope for us. And now don't get me distracted from this favorite topic: Eating Out.

Eating Out and Diabetes

Making healthy choices when eating out is a must. Notice that I didn’t add for the diabetics because it’s true. We all have to eat healthy but temptations are everywhere and that’s the problem. Especially if you’re like me, who loves to eat at buffets. I have a good reason for this, you know. When you order something at regular restaurants and you happen not to like the food, you’re stuck with it. Whereas at buffets, when you don’t like something you can go back and try something else. Does that make sense?

Okay no more excuses. We should really be careful with what we eat if we want to manage both the weight and the blood sugar level. I am not going to say it is easy but with self control, it can work out. It really is doable. And if you do this regularly, it becomes a habit and that’s when it becomes easy. Okay, let’s follow my journey at the buffets I frequent.

Let’s start with the drink. You know how they start the meals at any restaurant. The waiter asks your drink preference. You wouldn’t believe what I prefer to drink. Yes sir, it’s a glass of water, that’s my baby. I can’t even have a lemon with the water which I used to love so much. But you see, my body is acidic so lemon is a no, no.

Now the rush to the buffet table is on and what do you think do I choose for my appetizer? Two of them! Oops, you caught me red-handed, but let me explain before you condemn me without trial. Appetizer #1 is a salad and I don’t really like it that much, but I have to eat something raw at every meal. At least, that’s what my mother told me. Anyway, for the salad, I opted for a light dressing. So at least you should be proud of this girl for trying!

Now let’s go to the soup, my appetizer #2. It’s clam soup, my favorite. The chef at this restaurant makes this soup so good, I drool at the thought of it even in my sleep. No, it’s not the creamy Boston Chowder, that’s too heavy even for me, a buffet lover. It’s just a clear soup with real clams in it. Oh my gosh! I hope he didn’t put MSG in it! Oh, well, I don’t get a headache or other side effects from it. Besides, a girl has got to live and enjoy life sometimes.

And now, drum roll please, let’s go to the entrĂ©e table. This is easier than I thought. For one thing, I don’t eat beef. No, I don’t belong to that religion where beef is taboo because they think the cow is sacred. I don’t even know now if there is such a religion. But I digress, pardon me. I don’t eat beef because I am afraid of Mad Cow Disease.

I have no explanation as to why I am afraid of Mad Cow Disease for I was told it has an incubation period of 30 years; I will be long gone before that happens. But that’s not true really because I saw a teenager suffer from it. And since then I refused to eat beef, not even the beef from a cow bred with Japanese tender loving care and fed with beer. And that’s why chickens hate me.

So the choice is clear. It’s either fish or chicken. That fish looks so inviting and the chicken, well, it’s beckoning to me. So how can a girl resist those looks, you tell me. So I got both. Now you’re giving me a dirty look, you diabetes police, you! Don’t be so down on me. It’s just half a fish and a chicken wing, for crying out loud. Besides, I loaded my plate with a lot of vegetables, so there.

And now to the dessert table we go. It is an easy choice; I picked an assortment of fruits - apples slices, watermelon, melon, papaya, oh no, this is a GMO (genetically modified organism) so I left that in the serving tray. I did the same to the pineapple and oranges because remember I am acidic. Then I looked at my husband’s plate and saw this gorgeous-looking cheesecake, reminiscent of the Lindy’s Cheesecake we had in Manhattan.

I made a quick dash to the dessert table racing against my self-control and dove right in before my right mind gets the better of me. But this, I’ve got to tell you and you will be proud of me. I took a spoon and just cut a piece from the slice of cheese cake and while I was there, I also cut a spoonful out of the carrot cake and the blueberry pie and went back to my table mates happy as a clam. I savor each bit of those three desserts ever so slowly so it felt like I ate the whole piece and that I‘ve gone to heaven. Watch below a more disciplined person eating out.

6. Some Humour - Let's laugh together at the joke below even if it's not funny. I really want to make you laugh because I feel that laughter makes one feel better. With humor, you can entertain people but you have to remember a few things the first of which is you have to know what’s funny. And that’s where I think I fail. But at least I’m trying.

Caught Cheating!

Teacher: Hey Peter! What do you think you’re doing?.

Peter: Nothing, Ma’am?

Teacher: Nothing,? You opened your notes and copied them.

Peter: Ma'am, seeking help is not a sign of ignorance.

Teacher: You are really something else.

Peter: What I did was an intellectual act.

Teacher: Stay after school and I‘ll have a talk with you.

Peter: It made me think there are things that can’t be handled alone.

7. Q&A: How Do You Survive a Restaurant Meal?“

Answer: One really has to be careful when dining out. Think of ways to keep control like calling the restaurant ahead. Find out the different menu options they offer. They will be glad you called. Calling ahead will enable you to decide what to order that will best fit your needs. You will be comfortable knowing there will be something in the menu you can order.

If there is a kids’ menu, maybe you can order from there. Unless of course the children’s menu only comes with hotdogs, burgers and similar items. Failing this, you can request for a children’s size from the regular menu or a half portion. Ask questions for there maybe substitutions available. You can also checkout the nutritional information for the meal.

8. Got something to say? Please write down your questions and comments in Facebook.

Just go to the https://www.facebook.com/pages/Free-Diabetes-Alert/22286671840 Just copy and paste that on your browser. Facebook will want you to join me first before you can send me a message. Okay, I failed at this. Eating out is my Waterloo, especially when indulging in a buffet. But I will not let that get me down. I will rise above that, I promise. Didn't Babe Ruth strike out many times more than anyone else? Didn't Enrico Caruso fail to reach the high notes many times to the point his teacher told him to quit?

We should all try to imitate Thomas Edison whose teacher called him a dunce and to prove it, he failed 14,000 times trying to perfect the incandescent light! We should also try to emulate Walt Disney who went broke seven times and had a nervous breakdown before he became successful. These people succeeded because they kept at what they had to and that's what all of us will do!

Have You Seen This New “Points System” for Better Nutrition and Physical Activity?

There really is a good tool to help choose foods that are good for diabetes. And it is good for tracking the meals. What it does is calculate the food score and then you can easily find recipes, menu plans and meals in restaurants that will suit your particular needs and will be better for your blood sugar. You will find this tool at thrive365.com

They say Thrive 345 will make you fall in love with food again. How will this tool do it? By simplifying the process. The complicated way of tracking what we eat will soon be over because this tool will do the counting for us. We will be able to meet head on the challenge of eating right.
Don’t let diabetes take away your enjoyment when eating out. With the help of this tool, you will have control over what are your choices to eat before you even arrive at the restaurant. You can select the food you want to order in the menu. Then thrive 365 will score the items to check out if they fit your particular needs.

You will be able to choose what foods to eat on your way to the restaurant. You will be able to choose nutritious foods that are good for your blood sugar. Can you see how this will expand your selection of foods? You will be in the driver’s seat as you will have full control on what to eat.

This tool consists of two parts. The food scores will help you select the nutritious foods that are right for you. The beauty of this is that you can make the right selection whether you are at home, in the restaurants or at the grocery store. This is great because now I don’t have to bring a calculator as well.

Mind you, my ipod has a calculator so that helped me a lot because then I don’t have to dig it out of my purse while keeping an eye on people with evil intention of taking a photo under your skirt. What they can get out of that is beyond my understanding. But you still have to watch for that and other shenanigans as well. Hmm, may be those people should get diabetes so they can make better use of their time.

Thrive 365 I think also has Healthy Eating Guidelines that will help us balance the nutrients in our meals. They have patented this food scoring system. It is designed particularly for type 2 diabetics who are not dependent on insulin. I don’t know why the type 1s will not benefit from this either but they have it in mind. I can’t wait to get my hands on this but they are still on beta. But I am so excited over it that I just had to report it to you.

Being in beta is not a bad thing. That only means there are still features that are under development. I wish windows would be on beta longer whenever a new one comes around so I don’t have to squash some bugs. That happens to me a number of times whenever I buy a new computer and it always comes with a new window.

I always prefer the old window because we have already developed a rapport and so we kind of understand each other. Not with the new window. It does not like me and I don’t like it and many a time I have had to report why something does not work on my computer with the new window. Has that ever happened to you? Or is it just me being so technologically challenged?

Sorry to digress, but Thrive 365 consists of two key parts: food scores to help you make better food choices at restaurants, at home and at the grocery store and Healthy Eating Guidelines to help you balance the nutrients in your meals. It is a patented food scoring system designed specifically for Type 2 diabetic people who are not dependent on insulin.

Physical activity is a good partner to nutrition and you can keep track with this with the help of Thrive 365. Of course the first step is good nutrition. Then comes the next step which is physical activity. In this connection, Thrive 365 can keep track of the blood sugar, the exercise and the score. You will be well informed of your progress in both the eating and physical activity departments.

There are some quotations there that I like:

“An early morning walk is a blessing for the whole day.” Henry David Thoreau

“Food is for eating and good food is to be enjoyed.” Della Smith

“One cannot think well, love well, sleep well if one has not dined well.” Virginia Woolf

“There is no love sincerer than the love of food.” Bernard Shaw Now this one I don’t believe much because of an experiment that has nothing to do with this report but I just love to share it with you. If you put your wife and your dog inside the trunk of your car, and after awhile, you open the trunk, who do you think will jump up and down in joy at the sight of you? I rest my case.

Research to make Thrive365 available costs over $10 million dollars.
Developed by over 300 nutritionists who specialized in diabetes, they assessed what foods are most advantageous to the diabetics although this has not been studied with the type 1s but they have this in mind as well. Developed for your body type, this food scoring system is customized to make certain you have good nutrition for breakfast, lunch and dinner.

The thrive 365 is available at the website mentioned above and you, iPhone fanatics, will celebrate. Why? It is because your iPhone can scan the UPC code on selected food items at the supermarket and then the iPhone mobile application will automatically upload the scores. How cool is that!

Then just when you think everything in honky dory, it gives you an eye-popping revelation. It is integrated with the blood glucose meter called Telcare to track blood sugar automatically and of course, exercise cannot be left behind, heaven forbid. It is also integrated with Fitbit to track exercise automatically as well. Now I hate to think how much will all of these cost.

What Happens If You Lose Your Diabetes Device?

What happens when you lose that most important diabetes gear on your way to the country? Well it happened to someone when the clip of that Continuous Glucose Monitoring device gave way and went flying away never to be found again. That was sometime ago. Let's follow it up and see what happens. Will this person get her diabetes gear back?

She got help with the paper work to be able to file for a replacement. She got a rude awakening when she found out that her health insurance company would not cover the loss. Apparently anytime a diabetes gear is lost or stolen, it is not covered by the health insurance company. She was told it was an uncovered benefit, whatever that means.

Did you think this is the worse of this story? Well, think again because one year after when the warranty was up, the health insurance company again refused to replace her lost CGM. This is despite the fact that this person who lost her CGM gear through no fault of hers, has a history of hypoglycemia unawareness and a frequent visitor to the emergency room.

It looked like the health insurance company will never ever replace that CGM. Can you imagine that? It is difficult enough to have to deal with using these diabetes tools, but do we also have to be concerned about losing it? To this day that diabetes gear has not been replaced so this is something we have to live with.

The trouble is there is no specific insurance policy to cover these expensive diabetes gears. Cell phones can be covered by a specific insurance policy but not diabetes devices. That goes to show how low the diabetics are in the totem pole of health concerns and this pisses me off to no end.

What do cell phones have that diabetes tools don't have? What do cell phones do to save lives that diabetes devices don't? Are the diabetics such nobodies that they can't even try to protect the diabetes tools that can help them go on living? What have we become - just so materialistic that we turn away from the needs of our neighbors? We are not asking people to be like Mother Teresa for crying out loud!

If we can protect the cell phones, why can't we protect the very diabetic device that could help us from getting to the point of being between life and death situation? That said, let us check out how the policies of the vendors can impact us. This may not be the latest but I just hope there has been some improvement.

Insulet, the maker of the OmniPod does not offer insurance but carries a warranty for four years. They say very few people lose it and if you do, call them right away to buy a replacement which you will receive within the twenty-four hour period. This means you can't get held up while wearing it.

Medtronic does not offer insurance for stolen or lost diabetes devices either but they said this could be explored with the homeowners insurance. They may offer extra rider insurance like they do for jewelry. As for the warranty, they focused on what is not covered like if it is submerged in water. That typhoon Haiyan or hurricane Katrina better not come around.

Animas has a limited four-year warranty against defects by the manufacturing company. If your pump breathes its last while covered, the company has the option of replacing it with a new one or a used unit they call re-certified. They will not cover any loss from accidents, misuse by the user or any other parties.

Tandem does not offer insurance for the loss or stolen t:slim but added that renters insurance or homeowners insurance policies offer riders that could insure the t:slim. Are they counting these diabetes tools the same way they regard the jewelry? Well, I suppose these gadgets are more important than jewelry anyway.

Asante hinted at being the first to consider offering insurance for it. I can't wait but I bet there will be a long list of exclusions. Dexcom and Roche have yet to say anything on the matter but anyway as a rule the makers of CGMs and insulin pumps are not going to be responsible beyond the warranty period they are offering. It does not matter that our survival depends on these devices.

Let us take a look at our Homeowners Policies, shall we? Farmers Insurance does not cover insulin pumps while Allstate does but we are urged to check out with our agents if we have enough personal property limit to cover this. Nationwide covers this and treats it like our other personal property. They will cover for theft and damage as a result of a peril mentioned in the policy but not for mysterious loss and breakage.

To make sure if you're covered, talk to the agent. Make sure it is a schedule, that is listed in the list of properties that are specifically insured. Otherwise you may need a rider to cover it just like what they have for any high-value item. If it is on schedule, that means it is covered as long as the items do not go over the coverage limits.

There you have it. You now know the extent of the warranty coverage for your insulin pumps and other continuous glucose monitoring devices. It’s only against manufacturing defects. Otherwise we are on our own. While the homeowners and renters policies may have some coverage on this, the onus is on us to talk to the agent for clarification.

The Best Multivitamins for Diabetics

The diabetics cannot use the glucose for energy efficiently so the extra glucose stays in the blood. This could lead to high blood sugar making the diabetics vulnerable to develop complications. They don't give up however and they have tools in their arsenal to combat this challenge. What do they do?

Most of them eat healthy diets and try their best to lose weight. Some of them may need multivitamin supplementation though and this is why we are writing about this. Multivitamin supplementation may help a lot but we have to do it the right way. For one thing, talk to your doctor about this as he knows your over-all health condition.

What are the vitamins associated with diabetes?

Taking vitamin E may prevent damage to the kidney, eyes and heart as these are associated complications of diabetes. Joslin Diabetes Center says the extra 2000 mg of vitamin C may help with the problems the diabetics have when it comes to lower vitamin C levels. They say this may be due to the higher blood glucose levels that prevent the uptake of vitamin C.

The diabetics, especially the type 1s may have insufficient vitamin D so giving them a supplemental dose of vitamin D may make the use of glucose better and improve bone health in addition. It is also often recommended for the type 2 diabetics to take Chromium as this has been reported to lower the blood glucose level.

The diabetics have also been found to have lower lutein and lycopene levels so supplements on these may improve vision. In addition, these supplements will lower disease in the eyes. Then there are the supplements Omega-3 fatty acids that can help protect the diabetics against heart problems.

What is the best multivitamin?

Here are what a good multivitamin should contain. It should contain at least 50% to 150% of the daily value for each of the 15 kinds of mineral and vitamin:

Vitamin A

Vitamin B-complex (riboflavin, thiamine, niacine, B6 and B12

Vitamin C

Vitamin D

Vitamin E

Vitamin K

Calcium, chromium and copper

Folic Acid and iron

Lutein and lycopene

Magnesium and omega-3 fatty acids

Selenium

What are the recommendations as to dosage for men and women?

Of course a lot will depend on conditions, sex and age.

For Men:

vitamin A 900 mcg

Vitamin B-complex (riboflavin 1.3 mg, thiamine 1.2 mg, niacine 16 mg, B6 1.3 to 1.7 mg and B12 400 mcg

Vitamin C 90 mg

Vitamin D 600 to 800 IU

Vitamin E 15 mg

Vitamin K 120 mcg

Calcium 800 to 1000 mg, chromium 35 mcg and copper 700 mcg

Folic Acid 400 IU and iron 6 mg

Lutein and lycopene

Magnesium 350 mg and omega-3 fatty acids

Selenium 45 mcg

For Women:

It is the same amount as for the men for B6, B12, vitamins D and E, copper, calcium, folic acid, and selenium except for the following:

Vitamin A 700 mcg

Riboflavin 1.1 mg and thiamine 1.1.mg, niacin 14 mg

Vitamin C 75 mg.

Vitamin K 90 mcg

Chromium 25 mcg iron 8.1 mg of iron, zinc 6.8 mg

Magnesium 265 mg.

It should be remembered that that there are other factors to consider when taking supplements. Check with your doctor for sure because overdosing on supplements is a no, no. They can become toxic then and get in the way of your medications. Also this should not replace the balanced healthy diets that we should all eat.
